Before Your Appointment
To help us provide the best care, please bring the following items with you:
- A photo ID (driver’s license or other government-issued identification)
- Your insurance card(s) (Medicare, Medicaid, CHIP, or private insurance)
- A list of current medications, including prescriptions, vitamins, or supplements
- Any recent medical records or test results to share with your medical provider (if available)
Arriving 15 minutes early helps us complete your paperwork and keeps your appointment on schedule.
During Your Appointment
Your first visit typically includes:
- A review of your medical history and family health background
- A discussion of your current health concerns and goals
- Basic health checks such as blood pressure, temperature, and weight
- Time to ask questions and discuss preventive care or treatment options
Depending on your needs, your provider may order lab work or schedule ongoing screenings.
After Your Appointment
Following your visit, you may:

- Get instructions for prescriptions, lab work, or follow-up appointments
- Be connected with our health education programs or insurance counseling team if needed
