Partners & Resources
Through our trusted network of local partners, Barbour Community Health Association is dedicated to supporting your whole well-being—not just your medical care.

Dedicated to
Quality Care and Quality Caring
At Barbour Community Health Association, we believe caring for your health means supporting every aspect of your well-being. That’s why we work closely with a trusted network of local partners and community resources to connect you with the help you need—whether it’s food assistance, family and housing support, transportation, safety services, or counseling for life’s challenges.
Building a truly
Patient-Centered Medical Home
Because we’re building a truly patient-centered medical home, we work with partners across our communities.
At Barbour Community Health Association, our ongoing journey means more than providing clinical care, it means coordinating networks of support, partnering with community resources, and ensuring the full spectrum of health needs are met.
What is a Patient Centered Medical Home?
A PCMH is not a building, hospital, or nursing home — it’s a partnership between you and your primary care team. This model of care brings together physicians, nurses, and other health professionals who coordinate all aspects of your healthcare to ensure your needs are met in a comprehensive, compassionate, and efficient way. The PCMH approach emphasizes:
Comprehensive Care
Your care team works together to provide you with a full range of healthcare services, addressing your physical and behavioral health needs, preventive care, and chronic condition management.
Patient Centered
You are an active partner in your care. Our team listens to your concerns, respects your preferences, and supports you in achieving your health goals.
Coordinated Care
Your BCHA care team helps manage and coordinate all aspects of your healthcare — including specialist referrals, hospital transitions, home health services, and community support.
Accessible Services
We offer multiple ways to communicate with your care team, including same-day appointments, after-hours access, and reminders — ensuring you can reach us when you need care most.
Quality and Safety
Our team uses the latest evidence-based practices and advanced electronic health records to support informed healthcare decisions. We are committed to continuous improvement through patient feedback, quality measurement, and population health management.
At Barbour Community Health Association, we are proud to take this step toward PCMH accreditation — reaffirming our commitment to providing coordinated, compassionate, and high-quality care that keeps you at the center of everything we do.
